School success goes hand in hand with attendance and is very important here at CMHS. Please note that all absences will be coded unexcused unless this form or appropriate documentation is submitted to the school. Absence notes must be submitted within two (2) school days of the student’s absence and/or return to school. Notes will NOT be accepted after TWO (2) school days of the student's return.

*Automated calls from the County Office will be sent out if a student is marked absent or tardy in class*
Students who check in late or check out early (more than halfway through a period) will be marked tardy for the period.
**No check outs after 1:45pm**

College Visits and any trips longer than 3 days
This form is different than the regular absence form and needs to be filled out instead of the form above if you are on an official college visit.
Educational opportunity (Request must be submitted to Principal for approval.) Form is found under Attendance Tab and reads Education Opportunity Form. Please note absences for personal trips are not excused according to state guidelines. *Educational Opportunity exceptions are typically only granted to students who will be absent due to travel directly related to academics.*
FORM MUST BE COMPLETED BY PARENT/GUARDIAN ONLY. Email address must match Parent/Guardian email in PowerSchool for the form to be honored.

Attendance FAQ:
What is the latest time I can check my student out?
Cabarrus County policy does not allow check outs after 1:45pm (student drivers or students
being picked up by a parent/guardian), if the schedule changes for any reason, it is 30 mins
before school is dismissed.
My child has a scheduled appointment, or I am running late to pick him/her up, can he/she
wait in the front office for me to pick them up?
No, to preserve valuable instructional time, we will not call a child out of class to wait for their
parents in the Front office to be picked up. At the time of your arrival and completed check out
in front office we will call the classroom to have the student sent for check out. We apologize for
any inconvenience, please plan accordingly.
Is documentation necessary for the Absence, Early Check-outs/Late Check Ins for which
the excuse is Dr/Dental Appt is used? (Dr notes, hospital forms, court procedure excuse,
etc.)?
Yes, all absences will be coded unexcused unless the Absence/ Attendance form and appropriate
documentation is submitted for the absences that are allowable by state law. (Dr/Dental Appts
must still also provide a doctor’s note, court procedures-proper documentation, religious
observance explanation, covid, and death in family-funeral program/memorial pamphlet). You
may now conveniently upload all documentation on the attendance form. Form must be
submitted within 2 days of when the student returns to school.
What is Cabarrus County Policy regarding tardies/early check outs?
In HS, a student must be present in EACH class for at least 45 minutes, to be counted present for
that day. VERY IMPORTANT - Four (4) tardies (in any combination of early check-outs, late to
school check ins, or in school tardies) will equal 1 WHOLE absence. Please be mindful of this
new Cabarrus County rule when checking your student out or when a student checks in late.
My contact information has changed, and I have included my new number on this form, is
this allowed?
NO, your contact information MUST be up to date to be able to dismiss your child from school
–Please help us by keeping your emergency contact info updated. If you have changed contact
information, please visit our office to complete the form with updated contact info. We cannot
make changes via phone or through someone other than the parent/guardian. Please be sure the
person uploading the ID is the Parent or Guardian.
Can I just call in to check my Student Driver out of school?
A Parent/Guardian must contact the Front Office/Attendance Coordinator, we must speak with
you to verify specific information before we can release your student. Voicemails are not
honored to release them; we must speak with you directly.
Students who have more than eight (8) absences in a course during each ninety-day term will
NOT receive credit for that course. A student will be given one absence for each class when a
student has accumulated a combined total of four (4) unexcused tardies, check-ins, and/or
checkouts. (Example: 1 tardy + 2 check-in + 1 check out in 3 rd period will count as one
absence in that period). NOTE: this includes EXCUSED AND UNEXCUSED absences.
Contact student’s counselor or grad coach regarding student’s course credit.
Excessive absences may impact eligibility for participation in interscholastic athletics.

Any parent, guardian or other person violating the NC Compulsory Attendance Law can be
found guilty of a Class 1 misdemeanor.
