RETURNING   CCP   STUDENTS

Returning CCP students should complete all 3 steps below. Failure to complete all steps may result in your inability to retake CCP courses in the future. These steps are only for students who have been enrolled in CCP courses before.

Step 1: CMHS CCP Course Request Form

Step 2: CCP User Agreement Form

A document titled 'Career and College Promise (CCP) User Agreement' is displayed.

**Please note that CCP User Agreement Forms must be submitted at the start of EVERY YEAR**

Step 3: Flex Form

(If Applicable)

A FLEX Form document with text and fillable fields.

**Please note that Flex Forms must be submitted at the beginning of EVERY SEMESTER**
